New Patient Registration
We welcome the registration of any new patients residing in our area. You will need to download and fill in the document below. You will then need to bring these into the Surgery with two forms of ID. For more infortmation, please contact the Surgery.
Please note if you are registering babies or children at the Surgery, a parent or guardian must also be registered at the Surgery.
In addition the following documents are for your information
You will be registered on the list of a particular GP, however, you are welcome to seek medical advice from any GP or nurse of your choice. In the interest of consistency and continuity of care we would ask that, where possible, you consult with the same person for on-going issues.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
